Which insurances do you take?

At Park Avenue Physical Therapy, we accept most major insurances including:
Medicare
Blue Cross/Blue Shield (PPO)
United Healthcare
Allcare
Central California Alliance for Health
We also work with VA and Work Comp insurances.

How long will my appointments take when I am there?

The initial evaluation takes about an hour and a half (90mins).
Follow up visits can range from 45mins to 1 hour, depending on your PT program.

Do you treat my condition?

Physical therapists have years of university level training in anatomy, physiology, kinesiology
(study of joint movement), neurology, and many of the other medical sciences.
Our physical therapists have graduated from accredited physical therapy programs and are state
licensed. Depending on the year graduated, the physical therapy program is either a masters or
doctorate level degree.

What should I bring or wear to my appointment?

If this is your first visit to our office, your ID, insurance card, and referral/script (if we do not
have a copy) will be needed.
Depending on your condition or treatment area loose or comfortable attire is recommended to
promote better movement as well as provide access to the area during your treatment.
